    Metallodrug ranitidine bismuth citrate suppresses SARS-CoV-2 replication and relieves virus-associated pneumonia in Syrian hamsters.

    Ranitidine bismuth citrate suppressed SARS-CoV-2 replication, leading to decreased viral loads in both upper and lower respiratory tracts, and relieved virus-associated pneumonia in a golden Syrian hamster model. In vitro studies showed that ranitidine bismuth citrate and its related compounds exhibited inhibition towards both the ATPase and DNA-unwinding activities of the SARS-CoV-2 helicase via an irreversible displacement of zinc(II) ions from the enzyme by bismuth(III) ions.

    Risks for cold frequency vary by sex: role of asthma, age, TLR7 and leukocyte subsets.

    People with asthma cases reported more colds per year and had lower baseline TLR7 gene expression than controls. Associations between many variables and cold frequency differed between women and men. In women, high blood neutrophil counts, and younger age, but not exposure to children, were independently associated with more frequent colds. In men, low TLR7 expression and high CLEC4C gene expression were independently associated with more frequent colds. Poor asthma symptom control was independently associated with reduced TLR8 gene expression and high body mass index. In conclusion asthma, age and markers of inflammation and antiviral immunity in peripheral blood are associated with frequent colds. Interestingly, the variables associated with cold frequency differed between women and men.

    Suboptimal treatment response to anti-IL-5 monoclonal antibodies in severe eosinophilic asthmatics with airway autoimmune phenomena.

    A significant number of patients who meet currently approved indications for anti-IL5 mAbs show suboptimal response to them in real-life clinical practice, particularly if they are on high doses of prednisone. Monitoring blood eosinophil count is not helpful to identify these patients. The concern of worsening of symptoms associated with immune-complex mediated complement activation in a small proportion of these patients highlights the relevance of recognising airway autoimmune phenomena and this requires further evaluation.

    SARS-CoV-2 infection induces endothelial cell activation, hypercoagulability, and enhanced inflammatory reaction. • Immunothrombosis is a major contributor in the COVID-19 worsening process. • Males, citizens with obesity, diabetes mellitus, or arterial hypertension, and patients with cardiovascular disease are at high risk for severe COVID-19. • VAS proposes a COVID-19-oriented primary health care network for patients with vascular disease and cardiovascular risk factors (VD-CVR). • Adherence to the antihypertensive, antiplatelet, antidiabetic, and lipid-lowering treatment and prevention of VTE are essential for the decrease of the risk for COVID-19 worsening in patients with VD-CVR. • For hospitalized patients with VD-CVR and COVID-19, VAS recommends early administration of thromboprophylaxis with an intermediate dose of LMWH or UFH and a regular evaluation of the biological efficacy of the treatment.
